How does it work?

Step 1:  Let the front desk know that you are interested in getting your annual blood work completed.  You have the option to file the lab costs with your insurance through DLO or pay cash for the blood work to our office if you don't have insurance.

Step 2:  We will send orders to DLO on your behalf for your blood work.

Step 3:  You will go to DLO located at 1611 Main Street, Woodward, OK to have your blood drawn.

Step 4:  DLO will run your profile and fax results back to our office.

Step 5:  We will set up an appointment to discuss your lab findings with you in person or via phone, make nutritional recommendations if needed, and give you a copy of your results.

What is the cost for the Custom Blood Panel?

Insurance:  If you have health insurance that you would like to use, DLO will file for the blood work cost (according to your insurance pricing) with your specific plan.  You will need to take your insurance information with you to your appointment for filing purposes.  Your fee to Family Wellness Center will be $55.00 for review of the blood work results and necessary recommendations.  This portion of your charge will be a cash fee paid prior to ordering the blood work and will not be billed to your insurance carrier.

No Insurance/Cash:  The cost of the annual blood work panel through our office is $289.00.  This fee includes the $55.00 for review of the blood work results and necessary recommendations.  For the same tests, you would pay $480.00 at DLO without our pricing agreement.  THAT IS A SAVINGS OF $191.00! 

How often should I get blood work?

Family Wellness Center recommends you complete blood work annually.  If you will also be completing an annual prostate or gynecology exam, we recommend you complete your blood work tests 2 weeks prior to your appointment so you can take your results with you to your annual exam.  (NOTE: If you would like to get PSA tests completed at the same time as your Custom Panel, please let us know prior to ordering tests.  Additional charges will apply to tests beyond our Custom Panel.)

What if I would like to have tests completed that are not included in the Custom Panel?

Family Wellness Center is happy to order requested tests once you have discussed your concerns with Dr. Phillips.  Additional fees for tests will apply.

Can I order a UA?

Urine analysis tests can be ordered just as easily as the blood work.  Once you have discussed your issues with Dr. Phillips, we would be happy to order your test at a discounted fee.  A regular UA through DLO costs $49.00.  Our cost to you is $40.00, saving you $9.00 per UA.  (NOTE:  This fee is for cash payments to Family Wellness Center only.  If you prefer to file this lab with your insurance, DLO will file according to the pricing agreement with your insurance company.  There will be an additional $10.00 cash fee to Family Wellness Center for review of your lab results and necessary recommendations.)

Remember to bring

• A Test Order (requisition or prescription) completed by your doctor.
  If your doctor did not give you paperwork, then it has already been sent to us.
• Your insurance card. Most plans accepted.

Come prepared

• Wear sleeves that roll up easily.
• Fasting may be required for lipids (cholesterol, triglycerides, HDL) and glucose tests.
• Fasting means don't eat any food or drink anything other than water before your blood collection. Continue taking all medication as prescribed and drink plenty of water.
• Fasting is generally 8 hours, however check with your doctor about requirements.
• Glucose tolerance tests may take up to 4 hours. Allow adequate time for your visit.
• Follow any other instructions your doctor has given you.
